This PDF form can be filled out in two ways.
Using a Web Browser: For fillable PDF forms, you can open them in web browsers like Google Chrome, Microsoft Edge or Safari. Just click into the fields and type your information. After filling it out, save the form by clicking the download icon in Chrome or the save icon in Edge and email the form to AJGarger@nypilotcommission.org.
Using Adobe Acrobat Reader: If you have Adobe Acrobat Reader, you can download the form to your local computer first, then open the Adobe Acrobat Reader App and edit using Adobe Reader. After filling it out, save the form, save the form in Adobe Acrobat Reader and email the form to and email the form to AJGarger@nypilotcommission.org.
